About The Position

The Primary Care Capitation Population Health Program Support Specialist will play a central role in the design, implementation and oversight of the Medicaid Primary Care Capitation Pilot under the Rural Health Transformation Program. The Program Support Specialist will work closely with the RHTP internal teams, Regional Hubs, and participating practices. This includes coordinating practice engagement and technical assistance efforts, and supporting the integration of pilot activities with broader Medicaid initiatives.

Requirements

Bachelor's degree from an appropriately accredited institution and two (2) years of experience related to the area of assignment OR An equivalent combination of training and experience.

Nice To Haves

Knowledge of Medicaid policy and value‑based payment.
Basic knowledge of accounting and fiscal concepts sufficient to understand payment model design, pilot performance monitoring, and implementation oversight.
Ability to exercise judgment and discretion in establishing, applying, and interpreting policies, procedures, and communications.
Ability to establish and maintain effective working relationships with internal teams, providers, vendors, partners, beneficiaries, advocates, and other stakeholders, along with the ability to plan, assign, and guide the work of team members or project contributors when required for program needs.
Ability to contribute to the development of communications, training materials, implementation tools, and stakeholder education resources.
