About The Position

Frontier Direct Care delivers direct primary care (DPC) to employer-sponsored patient populations. Our providers manage a defined patient panel with an emphasis on continuity, preventive care, and chronic disease management — a model built around lower daily patient volume and longer visit times than traditional fee-for-service primary care. We're hiring Family Nurse Practitioners and Physician Assistants to serve as the primary clinician to provide support, treatment and management to patients. This is a full-scope, hands-on primary care role: you'll manage chronic disease, perform in-office procedures, and build long-term relationships with patients rather than cycling through high-volume, short visits.

Requirements

• Family Nurse Practitioner or Physician Assistant certification.
