About The Position
Bookmark Medical is seeking an experienced Primary Care Advanced Practitioner (NP or PA) to join our established Primary Care team in Mesa, AZ. This role involves providing comprehensive care to a diverse patient population, promoting health education, and collaborating with other healthcare providers. Requirements
- Graduate of an accredited NP or PA program; current certification and licensure
- At least one year of APP experience, preferably in primary care
- Strong clinical skills, collaborative mindset, and proficiency with EMR systems
Responsibilities
- Provide comprehensive care to a diverse patient population
- Conduct thorough physical exams, diagnose medical conditions, and develop personalized treatment plans
- Promote health education and disease prevention
- Order and interpret diagnostic tests and manage chronic conditions
- Collaborate with specialists and other healthcare providers to ensure coordinated care
- Maintain accurate up-to-date medical records and patient charts
- Stay current with medical advancements and best practices in Primary Care
