About The Position
Revere Medical is seeking an experienced Internal Medicine/Adult Medicine Advanced Practitioner (NP or PA) to join our established Primary Care team in Brockton, MA. This well-respected community practice includes two Physicians, serving patients across all ages in a supportive, patient-first environment. The role involves providing primary care to an existing adult and geriatric patient panel, diagnosing and managing acute and chronic conditions, conducting Annual Wellness Visits, developing personalized treatment plans, ordering and interpreting diagnostic tests, and collaborating with specialists and leveraging technology to deliver coordinated care.
Requirements
- Graduate of an accredited NP or PA program; current certification and licensure
- At least one year of APP experience, preferably in primary care
- Strong clinical skills, collaborative mindset, and proficiency with EMR systems
Responsibilities
- Provide primary care to an existing adult and geriatric patient panel
- Diagnose and manage acute and chronic conditions
- Conduct Annual Wellness Visits and develop personalized treatment plans
