About The Position

Responsible for each assigned Program Participant’s prevocational plan of service and required program documentation. Assist Person We Support to develop work skills and behaviors in order for them to reach their maximum prevocational potential. Provide daily interventions with the People We Support and work in cooperation with all program staff to promote Independence, Integration, Individualization and Productivity.

Requirements

  • Bachelor’s Degree in Human Service Field.
  • One-year experience working with the Developmentally Disabled population or in a vocational setting.
  • Must have a valid NYS Driver’s License with an acceptable driving history per the agency’s insurance requirements.
  • Authorized to work in the United States.

Responsibilities

  • Complete all documentation required for annual reviews, semi-annual reviews, and interim meetings.
  • Complete Prevoc ISP programs, Habilitative Plans, monthly notes, and six-month reviews of progress on Therap.
  • Complete T-logs and PSU data entries as necessary.
  • Complete ISP Plans and Personal Focus Worksheets as necessary in Therap.
  • Verify daily attendance records.
  • Check and monitor daily documentation that is entered in Therap by Prevocational staff.
  • Ensure that all Documentation is scanned and uploaded to Therap.
  • Complete Discharge documentation and admission documentation for services.
  • Identify and help address barriers that may hinder People We Support’s progress toward supported employment.
  • Meet with individuals supported, as needed, to discuss concerns they may have.
  • Monitor waiver pre-voc services to ensure accuracy and completeness. Complete all required documentation for monthly billing and submit.
  • Assist Person We Support to gain access to needed community services and supports.
  • Maintain communication with families, care managers, residential staff and QIDPs.
  • Complete DDP1 & 2, needs assessments and annual review/revision.
  • Attend and satisfactorily complete all required in-services.
  • Assist with protective oversight.
  • Complete daily documentation as necessary for supports and goals.
  • Complete annual file reviews.
  • Coordinate, schedule and complete paperwork for annual community assessments.
  • Depending on caseload, additional duties may be given.
