About The Position

The Prevention Case Manager provides case management, outreach, prevention education, and harm reduction services to individuals with substance use disorders and those at risk for HIV, Hepatitis C, sexually transmitted infections, and opioid overdose.

Requirements

  • High School Diploma or GED required
  • Minimum three (3) years of experience in human services required
  • Valid driver's license and reliable transportation required
  • Ability to work occasional evenings and weekends as program needs require

Nice To Haves

  • Bachelor's Degree in Human Services, Public Health, or a related field preferred
  • Experience in substance use disorder outreach, prevention, or education strongly preferred
  • Knowledge of HIV/AIDS, Hepatitis C, substance use disorders, overdose prevention, and harm reduction principles
  • Experience working with high-risk populations, including people who inject drugs and other disproportionately impacted communities
  • Strong organizational, written, and verbal communication skills
  • Ability to work independently and collaboratively within a multidisciplinary team
  • Strong engagement, outreach, and relationship-building skills
  • Demonstrated ability to provide equitable, culturally responsive care to diverse populations, including: People of color, People who use drugs, Individuals experiencing homelessness, Sex workers, LGBTQ+ individuals, Individuals recently released from incarceration
  • Bilingual abilities in Portuguese, Cape Verdean Creole, Haitian Creole, or other languages commonly spoken in the Brockton area are highly valued

Responsibilities

  • Provide short-term case management services to individuals with substance use disorders
  • Connect participants to medical care, behavioral health services, medication-assisted treatment (MAT), and community resources
  • Facilitate warm handoffs and supportive referrals to treatment and recovery services
  • Follow up with participants to assess service effectiveness and ongoing needs
  • Develop and document service plans, referrals, and support activities
  • Conduct community-based and street outreach activities
  • Provide harm reduction education, supplies, and prevention services
  • Enroll eligible participants and bystanders in the Narcan Pilot Program
  • Utilize Uber Health and other transportation resources to assist participants in accessing appointments and treatment services
  • Promote COPE Center services through outreach, education, and information dissemination efforts
  • Screen, interview, and counsel individuals at risk of HIV, STI, HCV infection, or opioid overdose
  • Collect specimens through venous blood draws, finger sticks, and urine samples for HIV/STI/HCV testing
  • Coordinate specimen submission and transportation to the State Laboratory as required
  • Provide referrals and linkage to HIV/STI/HCV testing and treatment services
  • Facilitate educational groups on topics including opioid overdose prevention, safer sex practices, harm reduction, and substance use prevention
  • Maintain accurate and timely documentation, data collection, and reporting requirements
  • Complete statistical reports and monthly program reporting requirements
  • Collaborate with the Program Manager to evaluate program effectiveness and participant outcomes
  • Attend meetings, conferences, trainings, and professional development activities related to HIV, HCV, STI prevention, substance use disorders, harm reduction, and public health
  • Build positive relationships with participants, staff, providers, and community partners
  • Follow all agency policies, procedures, and funding source requirements
  • Maintain safety standards and utilize universal precautions at all times
  • Perform other duties as assigned by the supervisor or designee
