About The Position

This position assists with implementing individualized family service plans and facilitates productive working relationships between families, agency personnel, and other involved service providers. The role involves providing in-home services, casework contacts, and child care behavior management. It also includes 24-hour on-call crisis intervention for families. The Prevention Case Manager is contracted through the Department of Social Services and manages all aspects of cases, specifically Article 10 cases.

Requirements

  • Bachelor's degree, preferably in human services or a related field required.
  • Minimum of one-year successful human service experience in a behavioral health setting, preferably working with Child Protective Services or other child welfare settings.

  • Demonstrate knowledge of behavioral treatment, social skills, and age-appropriate developmental milestones of youth.
  • Demonstrated ability to work with families in crisis to bring about positive outcomes.
  • Understanding of basic needs and developmental issues of children in a way that promotes growth and respects individuality.
  • Strong written and verbal organizational skills.
  • Demonstrated knowledge of documentation requirements of licensing and accrediting bodies.
  • Demonstrate the ability to form effective relationships with youth and families.
  • Ability to form collaborative working relationships with program staff, other agency staff, and staff from outside agencies such as Department of Social Services.
  • Valid driver’s license.

Responsibilities

  • Provide in-home services/casework contacts and child care behavior management.
  • Provide 24-hour on-call crisis intervention for families.
  • Assist families in identifying and obtaining appropriate clinical, recreational, financial, medical, or other support services designed to fully integrate the family with community-based supports in accordance with treatment plan.
  • Monitor school attendance and performance for each child on caseload in accordance with Department of Social Services recommendations.
  • Accompany families to scheduled appointments and provide or arrange transportation when necessary.
  • Act as a liaison between parents/families, Family Court, Probation, school officials, agency personnel, or other service providers as necessary/appropriate.
  • Assume lead Case Manager responsibilities when indicated, which could include service plan reviews, scheduling meetings, and coordination of services for family members in need.
  • Organize and/or actively participate in case conferences and case transfers.
  • Complete documentation per agency policy and procedure (e.g., log entries, mileage, medical, incident reports, intake, Plan Amendments, affidavits, court updates, etc.).
  • Attend agency in-service trainings, workshops, and seminars as appropriate or required by supervisor.
  • Take part in Quality Improvement training and demonstrate commitment and active participation in the agency's ongoing Quality Improvement Program.
