Under the direct supervision of the Program Manager, this position is responsible for coordinating the services and support available to consumers as outlined in the Lanterman Developmental Disabilities Services Act. The role involves evaluating consumer progress, ensuring consumer rights and dignity, maintaining contact with consumers and families, providing advocacy services, seeking out and utilizing generic resources, performing crisis intervention, coordinating assessments, and providing case coordination coverage when needed. The position also involves coordinating, implementing, and updating program processes, serving Medicaid Waiver cases, completing placement and liaison activities, acting as a liaison to community agencies, and preparing written reports. Additionally, the role may involve arranging for consumer placement in residential facilities, providing case coordination services in community care facilities, representing the regional center at case conferences, and acting as a program sponsor for new employees. The position also requires participation in quality assurance evaluations, completion of administrative requirements, purchase of service forms, and consumer file record documentation. The coordinator must ensure timely completion of case-related paperwork, coordinate the purchase of services, prepare cases for reviews, and complete Targeted Case Management documentation. The role requires flexibility to accept transfers, maintain interpersonal and community relationship skills, comply with safety plans, work cooperatively in a team environment, and utilize agency IT systems. Bilingual skills are a plus.
