As a Pre-Admission Coordinator, you’ll play a key role behind the scenes by reviewing referrals, coordinating admissions, and helping patients transition smoothly into home health services. This is a great opportunity for an LPN who enjoys organization, communication, and being part of a collaborative clinical team.

Requirements

• Active LPN/LVN license
• 1+ year of healthcare experience
• Strong communication and organizational skills
• Ability to multitask and problem solve in a fast-paced environment
• Basic Microsoft Office skills and ability to learn EMR systems
• Valid driver’s license and reliable transportation

Nice To Haves

• Home Health or Hospice experience preferred
• Bilingual English/Spanish preferred

Responsibilities

• Review patient records prior to admission
• Ensure clinical documentation is complete and accurate
• Coordinate with referral sources, facilities, and clinical teams
• Track pending admissions and discharge timelines
• Support smooth transitions into home health services
• Communicate with patients, families, and healthcare partners
• Utilize systems such as Homecare Homebase and Workday
