About The Position

Member of the care delivery team that encompasses the overall care of adults with health needs by acting as a coordinator for patients through an episode of care. Remotely provides professional nursing assessments and education following established standards and practices for patients preparing for procedures. Assumes responsibility and accountability for patients under their care and guidance. Provides remote communication to these patients through the therapeutic use of self, the nursing process, and other health care team members. Provides recommendations by following established protocols and coordinates and collaborates with other departments to ensure consideration of patients physical and emotional needs. The delivery of professional nursing care at Stormont Vail Health is guided by Jean Watson's Theory of Human Caring and the theory of Shared governance, both of which are congruent with the mission, vision, and values of the organization.

Requirements

  • Bachelors of Science: Nursing (BSN) Preferred
  • 2 years Ambulatory nursing, or medical surgical nursing experience.
  • Required Experience in a broad base of nursing.
  • Registered Nurse - KSBN Required
  • Workspace is a quiet and distraction-free allowing the ability to comply with all security and privacy standards.
  • Stable access to electricity and a minimum of 25mb upload and internet speed.
  • Dedicate full attention to the job duties and communication with others during working hours.
  • Abide by Stormont Vail’s Remote Worker Policy and will review and acknowledge the Remote Work Agreement annually.

  • Experience in telehealth or providing healthcare over the phone.

Responsibilities

  • Initiates communication with patients to discuss procedures, processes, and resources during the patient’s episode of care.
  • Efficiently documents medical information using the appropriate forms and/or electronic applications.
  • Utilizes EPIC to gather data as appropriate documenting all patient interactions.
  • Maintains and reviews patient records to determine actual or potential problems and issues and makes recommendations.
  • Tracks interventions and outcomes during the patient’s episode of care.
  • Guides patients through the health care system during the patient’s episode of care.
  • Evaluation of patients to determine that specific criterion is met for the procedure(s).
  • Identifies expected outcomes for a plan individualized to the patient and situation, developing a collaborative plan encompassing strategies to achieve expected outcomes.
  • Triages and refers incoming telephone calls.
  • Oversees appointment scheduling based on nursing recommendations for that episode of care.
  • Evaluates the patient’s progress towards achieving outcomes.
  • Performs efficiently in emergency patient situations following established standards of care.
  • Practices effective problem identification and resolution as a method of sound decision making.
  • Participates in system wide/department patient care quality improvement activities as well as standards development.
  • Participates in committee activities, attends huddles, staff meetings, mandatory training in compliance with departmental requirements.
  • Delegates tasks and duties in the direction and coordination of health care team members, patient care and department activities in accordance with the Kansas State Nurse Practice Act.
  • Seeks validation of knowledge base, skill level and decision making as necessary and assertively seeks guidance in areas of question.
  • Complies with all policies, standards, mandatory training and requirements of Stormont Vail Health
