The Postpartum APC (Advanced Practice Clinician) will perform complete histories and physicals, document in the medical record, and order and collect data using appropriate assessment techniques. The APP will serve as the first responder for patient status changes, orders, and situations requiring procedural and resuscitative interventions. They will assess for risks associated with the care of acute and complex chronically ill patients, including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions, and diagnostic procedures. The APC will collaborate with the interdisciplinary health care team in making diagnoses of acute and complex chronic conditions, manage diagnostic tests, and formulate differential diagnoses by priority. They will diagnose complications, order appropriate interventions, and formulate a plan of care by identifying expected outcomes and documenting a plan to address complex acute and chronic healthcare needs. The APC will utilize evidence-based practice guidelines and protocols in an individualized, dynamic plan of care, implement and modify the plan, and prescribe diagnostic strategies and therapeutic interventions. Advanced procedures may be performed consistent with privileges and competency validation. Documentation includes key components of patient progress via daily progress notes, transfer and discharge summaries, and/or clinic notes, meeting acute care compliance standards and capturing patient acuity. The APC will maintain ongoing communication and collaboration with the interdisciplinary health care team, facilitate communication with patients, families, and staff to promote continuity of care, and demonstrate professional practice behaviors including preceptor/mentoring, education, and instruction of graduate students. They will seek opportunities for active engagement in research and analysis of evidence-based practice, actively participate in Advance Practice Grand Rounds and specialty-specific meetings, and maintain CEUs and membership in a professional organization.
