About The Position

A Population Health Post - Acute Care Navigator works with primary care physicians, specialists, outcomes management, and other services to maximize health services and coordination for patients. This position requires a high frequency of contact with patients admitted to the hospital or in a skilled nursing facility. A Population Health Post-Acute Care Navigator will ensure the coordination and communication of a patient's treatment plan and general status to all care givers during all transitions of care. This position requires knowledge of the health care system and expertise to identify and implement improvement processes and the ability to direct and implement care coordination plans both in both in-patient and out-patient settings.

Requirements

  • Graduate from an accredited School of Nursing or Physical Therapy with a minimum of Bachelors Degree in respective field.
  • Experience 5 + years in recent in nursing or physical therapy field.
  • Computer and keyboard skills.
  • Current State of New Jersey license (or legal permission to work from the New Jersey Board of Nursing/Physical Therapy).
  • Current and valid driver's license and evidence of State required insurance.
  • Demonstrates effective interaction and communication (oral, writing, presenting) skills.
  • Ability to work accurately and pay attention to details, often changing from one task to another without loss of efficiency or composure.
  • Ability to function competently in stressful situations and a changing work environment related to changing patient needs, including working with patients with acute, chronic, and complex disease processes and those who are dying.
  • Ability to work cooperatively within the health-system; with patients and family members; and with multidisciplinary team members (i.e., physicians, Rehab, etc.).
  • Ability to utilize effective time management to set priorities, perform job related responsibilities, and respond quickly to emergencies.

  • Experience in a variety of patient settings is recommended.
