Population Health Scheduler - Accountable Care Organization - Days - MFT
About The Position
The Population Health Scheduler oversees annual wellness and preventative visit scheduling, maintains precise patient data and electronic health records, conducts population health outreach through various channels, addresses general inquiries, verifies insurance benefits, ensures timely payments, coordinate schedules, aids patient in navigating healthcare system and directs them to relevant departments for necessary information and care. This is a Monday-Friday position in the Population Health Call Center. This role provides telephone outreach and education to patients and families regarding wellness and preventive services, while also handling incoming calls and assisting patients with navigating the health system to access the care and resources they need.
