About The Position

Build your Career. Make a Difference. Presbyterian is hiring a skilled Population Health Program Manager to join our team. As a member of the Mobile Health Center department, this position is responsible for the overall planning, management and execution of various initiatives to support the Mobile Health Center department. Using project management methodologies, this position manages the design, development and implementation of initiatives in connection with department leadership and various operational leaders throughout the enterprise. This position will play a key role in the design and implementation of toolsets and approaches used within the organization which optimizes advanced program management methods to create a highly effective, customized approach to rapid implementation. Actively seeks out best in class, cutting edge methods from leading organizations and identifies how these methods can be optimized to achieve targeted outcomes. This position will lead multi-disciplinary design teams which leverage integration to maximize results while also weighing the practical considerations and potential barriers that exist when implementing programs

Requirements

  • Bachelor Degree, 6 years of additional experience can be substituted in lieu of degree
  • 4 years of application specific experience.
  • Five or more years of relevant industry specific experience demonstrating strong leadership skills and a proven ability to facilitate cross-functional teams.

Nice To Haves

  • Masters degree preferred but not required based upon relevant experience.
  • PMP certification preferred

Responsibilities

  • Responsible for leading the successful execution and sustaining efforts of complex, enterprise-wide initiatives.
  • Promote adoption of an overall Integrated Health Management approach across PHS.
  • Ability to be dynamic in the selection, prioritization, and redirection of initiatives and approaches used.
  • Develops organizational capacity for innovation, development of best practices and knowledge transfer of these assets.
  • Develops tools and standard approach documents and other materials to enhance the performance of multi-disciplinary teams.
  • Prepares senior level management, board, and program reports as directed.
  • Engages and influences internal (PHS executives, services lines, PCMH, PMG providers) and external (network providers, Government entities, community resources, customers) stakeholders.
  • Ensures financial and operational results are achieved for all Population Health initiatives.
  • Uses integrated decision making to ensure optimize outcomes.
  • Gathers data, analyzes metrics, and participates in the documentation and measurement of processes, work loading, turnaround time, and throughput analysis.
  • Participates as key team member in building the Population Health department with standardized assessments, documentation, tools, and comprehensive financial analysis.
  • Deploys comprehensive communication strategies to internal and external stakeholders.
  • Responsible for developing a complete working knowledge of the PHS organizational structure, leadership systems, measurement systems, and key processes
