About The Position

This clinical pharmacist will practice in the Office of Population Health and the Vanderbilt Health Affiliated Network, primarily supporting primary care clinics. The primary responsibility of this pharmacist is to provide support to patients, clinicians, and the care management team to ensure effective, safe, and cost-effective drug therapy in a population health model. The pharmacist in this role will interact with patients and providers remotely and face-to-face as needed. Additional responsibilities include patient and provider education, disease management, comprehensive medication reviews, transitions of care, motivational interviewing, precepting students and residents, and assisting in development of network resources and initiatives. We are seeking individuals with a patient-first mindset, excellent problem solving and communication skills, who can thrive in an autonomous and evolving environment.

Requirements

LIC-Pharmacist License - Tennessee
Relevant Work Experience
Less than 1 year experience
Bachelor's degree
