About The Position

Under supervision of Manager Population Health Pharmacy, assists the pharmacist team in caring for patients by providing point patient outreach services as assigned. Promotes the safe and effective use of medications by providing the clinical ambulatory pharmacist team with accurate and timely information necessary to make informed clinical recommendations related to medications and clinical quality measures. Job Description ESSENTIAL FUNCTIONS: Assists with a patient's medication management for chronic disease states by obtaining and organizing information such as patient demographics, medication histories, and insurance information. Demonstrates the ability to solve problems, recognize and correct potential errors in the various patient populations. With a primary focus on populations with the following chronic conditions: diabetes, hypertension, cholesterol management. • Act as a liaison between the patient, provider and ambulatory pharmacists in understanding insurance related matters, authorizations and medication directions as needed. Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Excellent prioritization and customer service skills are required. Interpersonal skills necessary for obtaining, verifying, and exchanging information with pharmacists, providers, practice staff and other participants in the patients care team. Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.

Requirements

  • High School or GED required, Associates or Bachelor degree preferred
  • Graduate of an accredited Medical Assistant or Pharmacy Technology Program
  • Pharmacy Tech certification (PTCB)
  • 2+ years work experience as a pharmacy technician in an ambulatory setting

  • Applicable experience as a pharmacist extender is desired.
