About The Position

Under the supervision of the Director of Population Health, the Population Health Outreach Specialist will be responsible for contacting patients who tested positive for COVID-19 via electronic lab reports and case reports submitted by healthcare provider. Primary responsibilities include, but are not limited to, coordinating with the Managed Care Organizations to collaborate on priorities and outreach efforts to improve compliance and decrease gaps in care.

Requirements

  • High school diploma. Associate degree preferred.
  • Two (2) years of related experience
  • Extensive experience with substance abuse, incarceration, homelessness and other issues facing out of care individuals.
  • Knowledge of DC community resources and services; ability to assess clients for needs related to treatment education, risk reduction and prevention.
  • Demonstrated documentation skills
  • Knowledge and understanding of health care terminology.
  • Ability to work both independently and as a team member
  • Demonstrated proficiency with business software (e.g. Microsoft Office Suite preferred).

Responsibilities

  • Calls patient for follow up appointments
  • Sends notification/T-CONS to providers of patients concerns
  • Sends certified letters to patients if unable to reach
  • Assigns lab reports to the Infection Control team
  • Schedules patients EPSDT appointments
  • Calls patient to inform them of lab results (Nurse only)
  • Connects patients lost to care to Primary Care Provider
  • Provides outreach to patients by visiting the home to engage to care
  • Develops a professional, therapeutic working relationship with clients.
  • Collaborates with Social Service staff to assist with case management needs related to addictions, mental illness, homelessness, domestic violence, pregnancy, family issues and on-going support.
  • Accurately documents and logs all patient encounters in eClinical Works (eCW) and responds to messages, referrals, and TCONS within established time frames per electronic medical record policy.
  • Participates in professional development opportunities.
  • Attends departmental, health center, providers meetings and mandatory trainings and meetings.
  • Assists Returning Citizens to navigate their health care
  • Performs other duties as assigned.
