About The Position
The primary purpose of a Population Health Coordinator is to improve the health outcomes of a population group by monitoring and identifying individual patient needs within the group.
Requirements
- 1-2 years in population health management or strong clinical background in nursing preferred
- Work experience in community outreach, data collection and analysis, Performance Improvement and Quality Assurance preferred
- BLS Required
- Healthcare software system knowledge and report building skills preferred
- Experience with quality improvement programs, integration and informatics related to planning and population health management.
- Knowledge of patient centered medical homes, Meaningful Use, MACRA/MIPS, and EMR based reporting.
- Skill in interacting and communicating with patients, physicians and care coordinators
