About The Position
This role focuses on population health within Value Based Care at Saint Thomas Health in Nashville, TN. The Population Health Coordinator will analyze patient records to identify gaps in preventative care, collaborate with providers to ensure clinical needs are met, lead patient outreach for necessary medical services, and drive quality outcomes by maintaining high healthcare standards.
Requirements
- BLS Provider preferred. American Heart Association or American Cross accepted.
- High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
- Primary care experience
- Experience working with Athena
- Medical office billing and front office experience
- Must be located in the Nashville area
Responsibilities
- Analyze patient records across multiple health databases to pinpoint missing preventative care.
- Collaborate with providers to validate clinical care needs against electronic medical records.
- Lead patient outreach initiatives to proactively connect individuals with necessary medical services.
- Drive quality outcomes by ensuring our clinic maintains exceptional healthcare standards.
