About The Position

Help make healthcare easier to navigate. The Care Coordination Aide supports CCP’s Population Health and Concierge Care Coordination team by helping members connect with the programs, services, and resources they need. This is a great opportunity for someone who enjoys working with people, solving problems, and making a difference in the healthcare experience. In this role, you will support care managers and the care coordination team by communicating with members and providers, assisting with referrals and outreach, gathering information, coordinating services, and keeping records accurate and up to date. You will also help with department projects, reporting, member education, and other activities that support quality care and member engagement.

Requirements

  • Associate degree or vocational certificate in healthcare, medical, social work, or a related field.
  • At least 2 years of experience supporting patients, members, clients, or customers within a healthcare environment.
  • At least 1 year of experience in managed care, health insurance, care coordination, case management, or a related healthcare setting.
  • Familiarity with Medicaid, Medicare, or other healthcare programs.
  • Strong communication, organization, and customer service skills.
  • Ability to manage multiple priorities while maintaining accuracy and follow-through.
  • Comfortable working with Microsoft Office and learning healthcare systems and technology.
  • Ability to handle confidential information with professionalism and discretion.

Responsibilities

  • Conduct member outreach and assist with connecting members to appropriate care coordination programs and services.
  • Support care managers by gathering non-clinical information and following up on member needs.
  • Assist members with navigating healthcare services, scheduling, transportation, and other barriers to care.
  • Review referrals and help identify members who may benefit from care coordination programs.
  • Maintain accurate documentation and tracking information.
  • Support reporting, quality initiatives, audits, and special department projects.
  • Collaborate with care managers, providers, and other teams to help create a positive member experience.
  • Participate in team meetings, training, and other activities that support the department.
  • Additional duties may be assigned based on the needs of the department.
