The Population Health Assistant will coordinate the intake of cases for new and established assignments to expedite the Care Coordination process. This role involves verifying health plan eligibility, obtaining and assessing clinical and non-clinical data, determining appropriate case types, and triaging to the correct resources. The assistant will also initiate and advance the Care Coordination plan, monitor its status, and provide guidance on requirements and processes. Additionally, the role includes researching and maintaining the team's resource library, performing medical literature review searches, and facilitating the implementation of new employer groups. The assistant will coordinate care gap outreach by generating lists of members requiring intervention and processing reminders according to established protocols, ensuring list accuracy and documenting interventions. Training and educating new employees and provider office staff on referral processes, contracts, and benefit plan limitations is also a key responsibility. The role involves developing and generating reports, gathering, compiling, and summarizing data for analysis, and assisting in monitoring and tracking data. The Population Health Assistant will also coordinate the development, ordering, and stocking of materials for Population Health activities, and oversee department files, binders, and records, establishing new record-keeping systems as needed. Routine administrative duties such as screening calls, managing mail, typing, monitoring office equipment, and purchasing supplies are also part of the role.
