A Day in the Life of a Population Health and Wellness RN The Population Health & Wellness Nurse is multifaceted-balancing clinical care with education and advocacy. Â It requires patience, empathy, and a strong knowledge of healthcare systems, particularly Medicare's coverage policies and how they apply to older patients. Throughout the day, the nurse ensures that patients not only receive medical care but also understand their health conditions and have the tools to manage them. This position involves direct responsibilities in developing and managing chronic care and wellness programs, focusing on program effectiveness and compliance with healthcare standards. Challenges include navigating complex healthcare environments and adapting to diverse patient needs while managing data for continuous improvement. Collaboration is key, requiring effective coordination with physicians, interdisciplinary teams, and community resources. The work structure typically varies, including clinic visits, patient outreach, program planning and community outreach efforts to promote health and wellness. Many patients may need extra guidance on managing chronic diseases, understanding new medications, or adopting healthier lifestyle habits. The nurse spends time discussing nutrition, exercise, medication adherence, and safety precautions, often incorporating information that is tailored to the patient's age and abilities. Our team works with primary care providers, specialists, and community resources to ensure that patients have the support, education, and care coordination they need to live healthier lives. Whether helping a patient better manage diabetes, access preventive screenings, or overcome barriers to care, we serve as advocates, educators, and navigators, ensuring that every patient receives the care they deserve. Requirements

  • American Heart Association BLS certification within 30 days of hire
  • Experience with inpatient and outpatient providers
  • RN license to practice in the State of Minnesota
  • Strong problem-solving skills
  • EMR expertise
  • Excellent interpersonal communication
  • Ability to multitask efficiently
  • Self-motivated and independent
  • Team collaboration skills

Nice To Haves

  • Three years clinical experience in the acute care and/or clinical setting
  • Bachelor of Science/Art in nursing field
