About The Position

The Population Health Advisor, IHCI, has responsibility for driving value-based care initiatives in Deaconess’s regional practices near and around Evansville, Indiana, Southern Illinois or Central Kentucky. This position works closely with the Quality and Analytics teams and helps integrate clinical programs, network saving initiatives, quality initiatives, and resources into the practice workflows with the intent to optimize the patient experience and positively impact physician engagement and quality of care provided. This role supports operational functions essential to IHCI’s success in value‑based contracts, focusing on closing quality care gaps, presenting insights to providers and health system leaders, and advancing network savings initiatives. The ideal candidate is relationship‑driven, analytical, adaptable, and passionate about improving patient and provider experience.

Requirements

  • Ability to communicate effectively with internal team members and key stakeholders.
  • Public speaking, PowerPoint, dashboards, visualizations, and/or other presentation formats.
  • Highly organized and self-motivated.
  • Ability to adapt to various workspaces and work autonomously.
  • Collaborative work style with the ability to work across different teams and areas of expertise.
  • Ability to adapt to ambiguous environments and clientele.
  • Bachelor’s degree or equivalent work experience.
  • Five (5) or more years of leadership experience in practice management.
  • Provider relations and project management experience.
  • Three (3) or more years of experience in practice re-design work, including Patient Centered Medical Home, Practice Transformation, Quality Improvement, ACO development, Ambulatory Care, Quality and Efficiency metrics, and Electronic Medical Records.
  • Health plan experience with a provider perspective and strong orientation to value-based care principles and the health policy landscape.
  • Experience in data analytics comprehension.
  • Valid Indiana Driver’s license.
  • Ability to travel regionally up to 50% of the work week.
  • Proficient in Microsoft Office products (Excel and PowerPoint).
  • Hypothesis-driven approach.
