About The Position

The Podiatrist provides comprehensive foot and ankle care to Rocky Mountain PACE participants as part of the interdisciplinary team (IDT), delivering preventive, routine, and specialized podiatric services on-site at the PACE clinic and, as clinically indicated, in participants' homes or contracted care communities. This role supports fall prevention, diabetic foot care, and mobility preservation for a frail, elderly population, and works in close coordination with primary care, nursing, and rehabilitation staff to promote participant independence, mobility and quality of life.

Requirements

Doctor of Podiatric Medicine (DPM) from an accredited school of podiatric medicine
Active, unrestricted license to practice podiatry in the State of Colorado
Completion of an accredited podiatric residency program
Current DEA registration, as applicable, and current CPR/BLS certification

Nice To Haves

Board certification, or board eligibility, preferred
Minimum two years of geriatric podiatric practice experience preferred
Experience working with geriatric, frail, or dually eligible (Medicare/Medicaid) populations strongly preferred
Prior experience in a PACE, long-term care, home health, or similar interdisciplinary care setting a plus

Responsibilities

Perform routine at risk foot care and medically necessary podiatric care, including nail care, callus/corn debridement, and management of common foot conditions in elderly and diabetic participants
Diagnose and treat foot and ankle pathologies, including diabetic foot ulcers, peripheral neuropathy complications, fungal infections, and structural deformities
Conduct comprehensive foot and lower extremity assessments as part of participant enrollment, semiannual assessments and ongoing care planning
Provide diabetic foot exams and preventive education to reduce risk of ulceration and amputation
Prescribe custom orthotics, therapeutic shoes, physical therapy, and medications to correct deformities and relieve joint pain
Perform minor in-office procedures, such as nail avulsion and wound debridement, within scope of practice
Participate as an active member of the interdisciplinary team, contributing podiatric assessments to individualized care plans and attending IDT meetings as needed
Document all encounters accurately and timely in the participant's electronic medical record in compliance with PACE and CMS documentation standards
Coordinate referrals to podiatric surgery, wound care specialists, vascular medicine, or other specialists when clinically indicated
Provide services at the PACE clinic and, when clinically indicated, via home visits or at contracted care communities
Maintain compliance with all applicable state, federal, and CMS PACE regulatory requirements
Participate in quality improvement initiatives related to falls, wound care, and diabetic management
Educate participants and team members on proper foot hygiene, preventive care, and condition management
