Requirements

  • Bachelor’s degree from a four-year college or university, or two to four years of related experience and training, or an equivalent combination of education and experience.
  • At least two years of experience providing direct services to families.
  • Computer literacy and familiarity with Microsoft Office products.
  • Ability to pass background screening including fingerprinting.
  • Strong understanding of maltreatment and non-maltreatment.
  • Excellent organizational and interpersonal skills.
  • Completion of 10 hours of Health and Safety Orientation Training within the first 90 days.
  • Pediatric First Aid and CPR Certification within 90 days of hire.
  • Comprehensive background check required.
  • Completion of 10 hours of annual training each year after the first year.
  • Knowledge of community wraparound services.
  • Experience working with individuals diagnosed with substance use disorders.
  • Effective communication and interpersonal abilities.
  • Basic leadership and decision-making capabilities.
  • Patience, empathy, and a positive attitude.
  • Ability to manage time and multitask in a fast-paced environment.
  • Team-oriented with a willingness to learn and adapt.
  • Three-year Motor Vehicle Report.
  • TB test and drug testing.
  • Background check.
  • Work performed in an office environment.
  • Standard work schedule is Monday through Friday, 8:00 a.m. to 5:00 p.m.

Responsibilities

  • Establishes rapport with family members of infants and their caregivers.
  • Responsible for establishing wraparound services that address the needs of the infant, caregiver, and family; enhance parenting skills; increase self-confidence; promote self-sufficiency; and reduce the chances of child abuse and neglect.
  • Responsible for initiating all referrals within 45 days.
  • Monitors progress toward identified goals and objectives of the plan.
  • Responsible for implementing and coordinating all services.
  • Maintains up-to-date, detailed documentation of all encounters with the family and service providers.
  • Maintains monthly data tracking forms to ensure timely and accurate entries.
  • Performs random urine drug screens and breathalyzer tests as needed.
  • Conducts home visits within the timeframe required by program guidelines.
  • Creates a Plan of Safe Care and conducts meetings to ensure the plan is adopted and carried out.
  • Facilitates and participates in regular monthly stakeholder calls and meetings to discuss process and qualitative outcomes.
  • Supervision of the family of the POSC plan.
