About The Position

Woolwich Community Health Centre (WCHC) is seeking a Physiotherapist for its New Hamburg site. This is a permanent part-time position, working 22.5 hours per week (0.6 FTE). The Physiotherapist will be an integral part of the inter-disciplinary team, responsible for assessing, developing, implementing, and evaluating treatment programs for clients with movement disorders, adhering to physiotherapy intake guidelines. The role also involves participation in small group education sessions as needed.

Requirements

  • BSc PT /MSc PT degree in Physiotherapy from an accredited institution
  • Registered and in good standing with the College of Physiotherapists of Ontario
  • 3- 5 years of clinical experience
  • Demonstrated ability to work effectively in a multi-disciplinary team environment
  • Excellent communication and interpersonal skills, including the ability to establish rapport and effective relationships with clients
  • Demonstrate a commitment to and knowledge of community-based health care, health promotion and illness prevention
  • Proficiency in clinical documentation and EMR systems
  • Access to a vehicle required

Responsibilities

  • Conduct comprehensive client assessment and develop personalized treatment plans
  • Deliver evidence-based physiotherapy interventions using various therapeutic techniques
  • Monitor and document patient progress, adjusting treatment plans as needed
  • Collaborate with other healthcare providers to ensure optimal client outcomes
  • Provide educational sessions of a preventative nature, including injury prevention strategies and self-management techniques.
  • Provide services in homes of clients when needed
  • Supervision of Physiotherapy Aide(s) or PTA
  • Contribute to WCHC’s quality improvement initiatives
  • Maintain accurate and detailed clinical records
