About The Position

The Calgary West Central Primary Care Network (CWC PCN) is a non-profit organization committed to a healthier future. As a Primary Care Network, we work alongside hundreds of family doctors (our members) to support the patient-doctor relationship and provide programs and services at no cost to doctor or patient. As the Physiotherapist, you’ll virtually assess patients referred by our physician members with a range of physical and functional concerns, and provide self-management advice, education, and referral/navigation support to CWC PCN contracted physiotherapy clinics and/or community programs. You’ll assess patient needs by gathering and analyzing information about the issue/concern from referral forms and through phone consultation with the patient. This is a part-time, temporary position (maternity leave contract), and the expected contract dates are early-mid September 2026 – October 1st, 2027. The schedule is Wednesday and Friday; however, there is the possibility of flexibility with the days of work. This role is primarily a work-from-home position with occasional in-person responsibilities; therefore, the successful candidate must be located within the Calgary Zone or the surrounding area (e.g., Chestermere, Airdrie, etc.).

Requirements

  • A bachelor’s or master’s degree in physiotherapy from an accredited university program is required.
  • A minimum of five years’ clinical experience in a variety of settings (e.g., acute, community).
  • The ability to manage a wide variety of clinical situations with varying levels of acuity, complexity, and diverse populations.
  • Active registration with the College of Physiotherapists of Alberta is required.
  • Proficiency in the Microsoft Office Suite (Word, Excel, Outlook, PowerPoint, Teams) and EMR systems (MedAccess, CHR).
  • A clear Criminal Background check with Vulnerable Sector check is required to start.
  • A current Health Care Provider CPR certificate (BLS Provider from the Heart & Stroke Foundation) is required.

Nice To Haves

  • A minimum of two years’ experience with the delivery of virtual or telerehabilitation interventions.
  • Experience with trauma informed care.
  • Certifications in diagnostic imaging and vestibular and concussion management are an asset.
  • Equivalencies of education and experience may be considered.

Responsibilities

  • Interpret assessment findings and other relevant information.
  • Work with the team’s program coordinator to book patients in for screening and assessment.
  • Interview patients to obtain relevant information about health conditions and personal and environmental factors.
  • Determine the patient’s expectations and their relevance to physiotherapy.
  • Develop physiotherapy diagnoses and plans for treatment.
  • Based on assessment and eligibility, refer patients to the appropriate services.
  • Provide self-management education to patients, as needed.
  • Maintain accurate patient records in an Electronic Medical Record (EMR).
  • Communicate referral outcomes and provide consult letters to referring physicians.
