About The Position

Signify Health, part of CVS Health®, is seeking a Part-time Physician to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships. The evaluations, lasting about 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can be diverse and may contain household pets, secondhand smoke, and other airborne allergens or irritants. Candidates must be able to perform duties comfortably and safely in these environments.

Requirements

Active, unrestricted license(s) in coverage area(s).
MDs and DOs.

Nice To Haves

Multi-state licenses.

Responsibilities

Provide In-Home Health Evaluations.
Connect one-on-one with patients and answer health questions.
Conduct medication and medical history reviews.
Perform physical evaluations.
Conduct additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry).
Use an iPad and an intuitive clinical workflow for each evaluation.
