About The Position

Signify Health, part of CVS Health®, is seeking a Part Time Physician to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations, lasting about 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Physicians will use an iPad with an intuitive clinical workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the patient's private residence, which may vary in conditions and may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to perform duties comfortably and safely in these environments.

Responsibilities

Provide In-Home Health Evaluations.
Engage with people in the comfort of their own homes.
Bridge gaps in care.
Build trusted relationships to make people healthier.
Conduct evaluations including medication and medical history review, and physical evaluation.
Perform additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry).
Use an iPad and an intuitive clinical workflow for each evaluation.

Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
