About The Position

Make a meaningful impact beyond the traditional clinic setting. Legacy Care is seeking a compassionate, clinically strong physician to provide PRN coverage for our Richmond-area post-acute care practice. This is an opportunity to deliver high-quality care with flexibility and autonomy while supporting medically complex adult and geriatric patients across post-acute and long-term care settings. You will work alongside advanced practice providers, facility leadership and interdisciplinary care teams to provide responsive, person-centered care in skilled nursing, rehabilitation, long-term care and assisted living environments.

Requirements

MD or DO with an active, unrestricted Virginia medical license
Active DEA registration
Board certification or board eligibility in Internal Medicine, Family Medicine or Geriatric Medicine
Experience in skilled nursing, long-term care, geriatrics, hospital medicine, primary care or post-acute medicine preferred
Strong clinical decision-making, communication and collaboration skills
Ability to work independently while partnering effectively with facility-based care teams
Ability to travel locally among assigned Richmond-area facilities

Responsibilities

Provide PRN coverage for assigned Richmond-area facilities
Deliver onsite primary, acute and chronic care to post-acute and long-term care residents
Evaluate changes in condition, acute concerns and complex clinical cases
Collaborate with nurse practitioners, nursing teams, facility leadership and other care professionals
Partner with patients and families to establish person-centered goals of care
Support appropriate utilization, quality improvement and regulatory compliance initiatives
Complete timely, accurate clinical documentation
Promote smooth transitions of care across the healthcare continuum
