About The Position

Signify Health, part of CVS Health®, is seeking a Full-time Physician to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships. The physician will conduct comprehensive and personalized health evaluations, including medication and medical history review, physical evaluations, and potentially diagnostic tests. The role utilizes an iPad with an intuitive clinical workflow and does not require prescribing medication, ordering lab tests, or altering current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions. Candidates must be comfortable working in diverse home environments, potentially including pets, secondhand smoke, and airborne allergens.

Requirements

Active, unrestricted license(s) in coverage area(s)
MDs and DOs
Able to cover the following primary locations: Syracuse and surrounding area

Responsibilities

Provide In-Home Health Evaluations.
Conduct virtual visits as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
Travel locally and within licensed states, as needed.
