About The Position

Altais is seeking a Physician to join their team-based primary care model. This role is responsible for managing direct patient care, implementing evidence-based medicine, and adhering to practice guidelines. The Physician will act as a patient advocate, coordinate healthcare services, and promote effective communication with patients. This position will also drive clinical initiatives, collaborate with health professionals, and utilize consultations or referrals as appropriate. The ideal candidate will focus on health promotion, disease prevention, health maintenance, counseling, patient education, Annual Health Assessments, and Chronic Care Management. They will provide direct and indirect patient care, identify risks, and collaborate with the interdisciplinary team to meet patient needs. The use of the Electronic Health Record (EHR) is crucial for managing patient care and clinical compliance. The role requires providing 24/7 call coverage or participation in a call group.

Requirements

  • Providing patient-centric care; serving as patient health advocate.
  • Working in interdisciplinary teams and employing evidence-based practice.
  • Applying quality improvement.
  • Ability to function independently as well as part of a team in a dynamic environment and quickly adapt to changing circumstances and procedures.
  • Excellent communication skills and ability to adjust communication styles when working with patients, their families, and colleagues.
  • Highly organized, strong time management skills and flexibility to manage multiple timelines and activities.
  • Board certification must be current.
  • Minimum of 1-2 years clinical experience.
  • Successfully completed residency in their specialty.
  • Active and in good standing Medical Licensure in the state of California and Board Certified.
  • Active and good standing NPI number.
  • Active and good standing DEA number from US Drug Enforcement.

Nice To Haves

  • 3+ years professional experience working as a PCP.
  • Significant experience working with an EHR, preferably Epic.

Responsibilities

  • Focus on health promotion, disease prevention, health maintenance, counseling, patient education, Annual Health Assessments (AHA), and Chronic Care Management (CCM).
  • Provide direct and indirect patient care as part of the treatment regime, identifying actual or potential risks based on patient screening data.
  • Collaborate with providers, ancillary personnel, community resources, and other interdisciplinary team members to proactively resolve patient needs.
  • Leverage the use of the Electronic Health Record (“EHR”) to manage and improve patient care, workflows, and clinical compliance.
  • Provide patient advocacy in the health care system to accomplish cost-effective care by coordination of health care services.
  • Promote effective communication with patients and encourage the role of the patient as a partner in health care.
  • Provide call coverage for assigned Brown & Toland members 24/7, 365 days a year or participate in a call group.
