About The Position

We are seeking a dedicated and compassionate Physician to join our Charlottesville team on a PRN basis. This role provides an opportunity to deliver high‑quality, patient‑centered medical care in post‑acute settings, including long‑term care and assisted living facilities, with flexible scheduling to accommodate clinician availability.

Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution
  • Active, unrestricted Virginia medical license and current DEA registration where required for prescribing
  • Board certification or eligibility in an appropriate specialty (family medicine, internal medicine, geriatrics or equivalent) preferred
  • Proven proficiency with Electronic Medical Record systems and timely documentation practices
  • Excellent clinical judgement, therapeutic communication skills and the ability to work autonomously while collaborating within teams

  • Experience in post‑acute care, long‑term care, geriatrics or similar settings preferred

Responsibilities

  • Provide comprehensive, evidence‑based medical care within post‑acute and long‑term care settings on an as‑needed (PRN) basis
  • Perform timely assessments, develop and implement person‑centred treatment plans, and manage ongoing care
  • Prescribe and manage medications in accordance with Virginia regulations; conduct medication reconciliation and monitoring
  • Collaborate with nursing staff, facility leadership and interdisciplinary teams to support resident safety, care transitions and positive outcomes
  • Document patient encounters accurately and promptly in the Electronic Medical Record (EMR), including clinical reasoning, risk plans and follow‑up
  • Provide education and support to residents, families and facility staff regarding care plans, medication effects and strategies for safe, dignified care
  • Participate in case reviews, care conferences and quality improvement activities as requested
