About The Position

The Internal/Family Medicine Physician provides high-quality, patient-centered primary care within a Federally Qualified Health Center (FQHC) using a team-based, patient-centered medical home model. The physician delivers comprehensive evaluation, diagnosis, and treatment to a diverse patient population and contributes clinical leadership within the organization.

Requirements

  • Board Certified or Board Eligible.
  • Graduation from an accredited school of medicine.
  • Completion of a residency program in a primary care area such as Internal Medicine or Family Practice.
  • Current DEA certificate.
  • Current or willingness to obtain hospital admitting privileges and credentials (as set forth in the Employment Agreement).
  • Current insurance privileges and credentials.
  • Valid Ohio driver's license.
  • Current BLS Certification or obtain within 3 months of hire.

  • Serves as the collaborating/supervising physician for advanced practice providers, including nurse practitioners and physician assistants, in accordance with state and federal regulations.

Responsibilities

  • Provide comprehensive assessment, diagnosis, and treatment of acute and chronic illnesses and conditions.
  • Prescribe, dispense, and administer medications consistent with clinical standards and regulatory requirements.
  • Maintain accurate, timely, and compliant medical documentation using a problem-oriented medical record and electronic health record (EHR) system -(NextGen).
  • Coordinate referrals to specialty providers and ancillary services as clinically indicated.
  • Educate patients and families regarding diagnoses, treatment plans, preventive care, and safety, ensuring adherence to HIPAA and other regulatory guidelines.
  • Receive and address abnormal test results during on-call periods or after hours when providing scheduled call coverage.
  • Provide emergency or urgent medical care within the scope of practice.
  • Initiate, adjust, and monitor medical regimens in accordance with evidence-based guidelines.
  • Provide ongoing follow-up, continuity care, and chronic disease management.
  • Contribute professional medical expertise to internal educational activities and clinical discussions.
  • Provide consultation to nurse practitioners and physician assistants as assigned.
  • Participate in the education of medical students, residents, or other learners as assigned.
  • Support collaborative, multidisciplinary care with clinicians, behavioral health providers, care managers, and other team members.
  • Assist in developing and delivering in-service training, staff education, and orientation related to patient care.
  • Contribute to clinical evaluations, performance improvement initiatives, and program assessments.
  • Assist the Medical Director and leadership team in the creation, review, and implementation of clinical policies, procedures, and protocols.
  • Provide input on scheduling, workflow, and operational enhancements that impact patient access and service delivery.
  • Adhere to all organizational policies, procedures, compliance standards, and professional guidelines.
  • Participate in quality improvement and quality assurance activities, including periodic chart audits and peer review processes.
  • Participate in community outreach, health promotion, or public health initiatives as requested.
  • Attend monthly staff meetings, clinical conferences, or required provider assemblies.
  • Maintain flexibility in scheduling to support clinic coverage needs, including occasional evening hours.
  • Participate in the on-call rotation in accordance with organizational call schedules- up to 3x/year for one per call rotation.
