About The Position

Signify Health, part of CVS Health®, is seeking a Full Time Physician to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The physician will conduct comprehensive and personalized health evaluations, including medication and medical history review, physical evaluations, and potentially diagnostic tests. The role utilizes an iPad with an intuitive clinical workflow and does not require prescribing medication, ordering lab tests, or altering current treatment regimens. The physician will be part of a national network dedicated to improving health outcomes and connecting people to the right care for their unique needs. Flexibility to travel locally and within licensed states is required, and virtual visits may also be conducted. The primary workplace is the private residence of health plan members, which can vary in conditions, and candidates must be comfortable performing duties in homes with potential allergens or irritants.
