About The Position
TeamHealth is seeking a quality-driven physician to join their post-acute care team in Farmington Hills, Michigan. This is a full-time opportunity (5 days/week) rounding in skilled nursing facilities, offering excellent support and training, autonomy, and earning potential. The role involves providing high-quality care to a diverse patient population, collaborating with an experienced multidisciplinary team, and building meaningful relationships with patients and their families. The position offers a comprehensive 3-month onboarding program, a dedicated clinical field mentor, and support from industry-leading clinical experts in post-acute medicine. Growth and leadership opportunities are available within the market. Innovative population health data reports will guide patient care, and the practice is part of TeamACO, a long-term Accountable Care Organization participating in the Medicare Shared Savings Program. The Medical Director Leadership Academy (MDLA) will provide training in quality improvement initiatives and collaboration with facility leadership.
Requirements
- Current physician license (State of Michigan) and DEA
- Strong clinical foundation and knowledge of healthcare regulatory standards
- Excellent communication and interpersonal skills
- Collaborative, team-oriented approach with a positive outlook
Nice To Haves
- Experience in post-acute, acute, ED, or clinic settings preferred, but not required; new grads welcome to apply
Responsibilities
- Perform comprehensive patient assessments, including medical histories and physical exams
- Develop, implement, and adjust individualized care plans based on patient needs
- Monitor progress and coordinate care to optimize outcomes
- Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach
- Educate patients and families on health management and preventive strategies
- Maintain accurate, timely medical records in compliance with regulations
