About The Position

This position involves providing comprehensive patient care within a primary care setting. The physician will be responsible for patient assessments, direct and indirect patient care, and coordinating with other healthcare professionals and services. A key focus will be on patient education, care coordination, and ensuring adherence to professional standards and protocols. The role also emphasizes pain management expertise and facilitating important patient documentation like POLST and advance directives.

Requirements

  • Assessment of the patient and direct and indirect patient care delivered in accordance with appropriate licensing board, hospital policies, procedures and protocols, and all other professional standards of care.
  • Systematic collection of patient assessment data, focusing on the physical, psychosocial, age, cultural, and spiritual needs of the patient.
  • Performance of systematic physical exam and history of present illness.
  • Documentation of assessment data, history of present illness, and physical findings on the appropriate medical record within established time frame.
  • Involvement of the patient/family and other healthcare providers when appropriate.
  • Facilitation of clarification of patient and family goals of care.
  • Development of action plans required to meet goals.
  • Involvement of ancillary services as needed.
  • Participation in the assessment of the patient’s readiness to learn, identifying the educational barriers and tailoring the educational process to meet the needs of the patient/family.
  • Ensuring the process includes education about safe and effective use of medications, pain and managing pain, available resources, obtaining further care and their responsibility for their care.
  • Acquisition and distribution of educational resources tailored to the patient's needs.
  • Provision of care coordination.
  • Working with all members of the health care team.
  • Utilization of outside entities including but not limited to spiritual care when appropriate.
  • Provision of care information at times of all transitions to the next provider.
  • Facilitation of completion of POLST documents as appropriate.
  • Facilitation of completion of advance directive documents as appropriate.
  • Provision of expertise in pain management with attention to equianalgesic dose management and non-medicinal methods of pain management.

Responsibilities

  • Refer patients to medical specialists, social services, or other professionals as appropriate.
  • Ensure patient assessment and care align with licensing board standards, hospital policies, procedures, and professional standards of care.
  • Collect patient assessment data systematically, considering physical, psychosocial, age, cultural, and spiritual needs.
  • Perform systematic physical exams and document patient history.
  • Document assessment data, history of present illness, and physical findings in the medical record within the established timeframe.
  • Involve patients, families, and other healthcare providers in care planning.
  • Facilitate clarification of patient and family goals of care and develop action plans.
  • Involve ancillary services as needed.
  • Assess patient readiness to learn, identify educational barriers, and tailor educational processes.
  • Educate patients and families on medication use, pain management, available resources, and future care responsibilities.
