About The Position

Altais is seeking a Physician to join their team-based primary care model. This role is responsible for managing direct patient care, implementing evidence-based medicine, and adhering to practice guidelines. The Physician will act as a patient advocate, coordinate healthcare services, and promote effective communication with patients. This position will also drive clinical initiatives, collaborate with health professionals, and utilize consultation or referral as appropriate. The Physician will leverage the Electronic Health Record (EHR) to manage patient care and improve workflows. The role involves health promotion, disease prevention, health maintenance, counseling, patient education, Annual Health Assessments (AHA), and Chronic Care Management (CCM).

Board certification must be current.
The PCP is required to provide call coverage for assigned Brown & Toland members 24/7, 365 days a year or participate in a call group.
Minimum of 1-2 years clinical experience
Successfully completed residency in their specialty
Active and in good standing Medical Licensure in the state of California and Board Certified
Active and good standing NPI number
Active and good standing DEA number from US Drug Enforcement

Nice To Haves

3+ years professional experience working as a PCP
Significant experience working with an EHR, preferably Epic

Responsibilities

Provides direct and indirect patient care as part of the treatment regime, identifying actual or potential risks based on patient screening data.
Collaborates with providers, ancillary personnel, community resources and other interdisciplinary team members to proactively resolve patient needs, including collaborating with other health professionals, and utilizing consultation or referral as appropriate.
Leverages the use of the Electronic Health Record (“EHR”) to manage and improve patient care, workflows and clinical compliance.
Provides patient advocacy in the health care system to accomplish cost-effective care by coordination of health care services.
Promotes effective communication with patients and encourages the role of the patient as a partner in health care.
