About The Position

Help ensure patients receive the coordinated, compassionate care they deserve. As a Physician Services Clinical Coordinator, you'll serve as the central point of connection between providers, patients, caregivers, and healthcare partners, helping to navigate complex healthcare needs and ensuring care plans are carried out smoothly and efficiently. This role is ideal for someone who thrives on organization, communication, and problem-solving while finding purpose in supporting elderly and medically complex patients. You'll coordinate services, facilitate communication between care providers, manage patient needs and follow-up activities, and help patients and families access the resources necessary to support their health and well-being. Working closely with providers and community partners, you'll play a key role in ensuring continuity of care while serving as a trusted resource for patients, caregivers, and healthcare professionals. Through your attention to detail, proactive coordination, and commitment to exceptional service, you'll help create a seamless healthcare experience that allows patients to receive the right care, at the right time, from the right resources.

Requirements

  • High school diploma or GED
  • CENA certification, MA certification, or equivalent work experience
  • Minimum 1 year of experience in medical field or medical office setting
  • Strong grasp of medical terminology
  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint)
  • Reliable transportation with valid automobile insurance
  • Excellent verbal and written communication abilities
  • Exceptional organizational and time management skills
  • Strong problem-solving skills with creative thinking
  • Ability to work well under stress and in urgent situations
  • Professional communication skills across all organizational levels
  • Understanding of communication barriers (hearing loss, cognitive impairment)
  • Ability to triage and prioritize effectively
  • Customer service orientation with empathy for elderly patients

Nice To Haves

  • Associate's degree
  • Experience with geriatric population and/or caregivers
  • Previous experience in care coordination or medical office management
