Physician, Per Diem Palliative Care [Ambulatory] (Oakland, CA)
About The Position
Sutter East Bay Medical Group (SEBMG) is recruiting for a Board Eligible or Board-Certified Ambulatory Palliative Care Physician. This role involves providing Palliative Medicine consultation services and participating in the Palliative Supportive Care Services interdisciplinary team. SEBMG, founded in 2005, is a growing multi-specialty medical group affiliated with Sutter East Bay Medical Foundation. The foundation provides operational and administrative support, allowing clinicians to focus on patient care. SEBMG comprises over 400 clinicians across more than twenty specialties, dedicated to serving the communities in the San Francisco East Bay Area. The physician will provide Palliative Medicine consultation services in various settings including home, SNF, RCFE, hospital, and office-based clinics (Palliative Care, Cancer Center, and embedded primary care clinics). Responsibilities include coordinating with interdisciplinary team members (NP, LCSW, RN, chaplain, care coordinator), facilitating access to supportive care services, coordinating care with inpatient palliative care colleagues, referring providers, outside agencies, and local hospices. Referral criteria are clear, excluding chronic pain management. The role also involves assisting in the development and growth of the outpatient program through quality improvement projects and educational outreach.
Requirements
- Fellowship in Hospice and Palliative Care Medicine
- Board-certified or board-eligible in Hospice and Palliative Care Medicine
- CA physician license or eligible for CA physician license
Responsibilities
- Provide Palliative Medicine consultation services in multiple settings: Home, SNF, RCFE, Hospital and Office (Palliative Care, Cancer Center, and embedded primary care clinics).
- Coordinate with interdisciplinary team members - each team has an NP, licensed clinical social worker, RN, chaplain, and care coordinator
- Facilitate access to appropriate supportive care services.
- Coordinate care with inpatient palliative care colleagues, referring providers, outside agencies and local hospices.
- Assist in the development and growth of our outpatient program by means of quality improvement projects and educational outreach
