The Nephrologist is responsible for providing expert consultation and continuing care for patients with renal-related conditions, including chronic kidney disease, acute kidney injury, and hypertension, while offering leadership to the comprehensive care team managing complex medical issues. The physician works collaboratively within a multidisciplinary team to deliver high-quality, patient-centered care that aligns with Central Health’s mission to improve the health and well-being of the communities we serve. The role may also include supervising advanced practice providers and serving as an attending physician for learners at various levels. Preferred candidates will have experience in FQHC or community health settings, demonstrate cultural competence in caring for diverse populations, and excel in preventive care and patient education. Familiarity with value-based care models, EHR systems, and telehealth is highly desirable, as is a commitment to addressing social determinants of health and reducing health disparities. Central Health's mandate is to serve as the safety net for individuals in the county who need healthcare services.
