About The Position

Provide direct primary medical care to all participants of the program. This role involves performing comprehensive history and physicals on new referrals, as well as interval history and physicals on established participants. The physician will evaluate and treat participants with episodic illnesses, refer them to medical specialists as indicated, and manage the care of participants admitted to the hospital or residing in nursing homes. This includes providing primary care, coordinating discharge planning, making regular visits to nursing homes, and performing telephone contacts with nursing home staff. The physician will also actively participate in family conferences and team meetings.

Requirements

• Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) from an accredited school required.
• At least 2-3 years’ experience.
• Minimum one year experience working with a frail/elderly population.
• Current license to practice medicine in SC.
• BLS Required.
• Proven clinical leadership.
• Ability to get along with others in a team environment.

Responsibilities

• Performs comprehensive history and physical on new referrals.
• Performs interval history and physicals on established participants completed quarterly or as indicated.
• Evaluates and treats participants with episodic illnesses.
• Refers participants to medical specialist as indicated.
• Admits participants to the hospital, providing primary care (attending) responsibilities for management, providing updates to the team, judiciously utilizing specialty consultants, and coordinating discharge planning with the team.
• Manages all care of participants in the nursing home, providing regular visits, performing telephone contacts with nursing home staff as required, and admitting nursing home participants to hospital when necessary.
• Actively participates in family conferences.
• Participates in team meetings, committees and work groups as assigned.
