About The Position

Provide direct primary medical care to all participants of the program. This role involves performing comprehensive history and physicals on new referrals, as well as interval history and physicals on established participants quarterly or as indicated. The physician will evaluate and treat participants with episodic illnesses and refer them to medical specialists as needed. Responsibilities also include admitting participants to the hospital, providing primary care (attending) responsibilities, updating the team regularly, judiciously utilizing specialty consultants, and coordinating discharge planning. Additionally, the physician will manage all care of participants in the nursing home, including regular visits, telephone contacts with nursing home staff, and admitting nursing home participants to the hospital when necessary. Active participation in family conferences and team meetings, committees, and work groups is also expected.

Requirements

  • Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) from an accredited school required.
  • Work experience may NOT substitute for education requirement.
  • At least 2-3 years’ experience, minimum one year experience working with a frail/elderly population.
  • Current license to practice medicine in SC; BLS Required.
  • Proven clinical leadership.
  • Ability to get along with others in a team environment.

Responsibilities

  • Manages all care of participants in the nursing home: (a) Provides regular visits; (b) Performs telephone contacts with nursing home staff as required; (c) Admits nursing home participants to hospital when necessary.
  • Actively participates in family conferences.
  • Participates in team meetings, committees and work groups as assigned.
