As a home care hospice physician, you will use your compassion, communication skills, and deep expertise to build meaningful relationships with patients and families, providing the highest-quality care at the most challenging point in life. You will see the person behind each prognosis and create care plans centered around the patient’s wishes, needs, and beliefs, often working in concert with the patient’s primary care physician. With a strong sense of commitment, you will advocate for your patients, discussing goals for their care while addressing their hopes and fears. You will live the hospice mission, seeking teachable moments everywhere, and educate patients, caregivers, and the communities you serve on the benefits of compassionate, dignified end-of-life care.

Requirements

  • Must be licensed to practice medicine or osteopathy in the state in which their program operates.
  • Board-certified or board-eligible in hospice and palliative medicine, internal medicine, family practice, or a related specialty.
  • Familiar with the principles of hospice.
  • Registered with the DEA.
  • Possess Commitment, Compassion, and a Can-do Attitude.

Responsibilities

  • Build meaningful relationships with patients and families.
  • Provide the highest-quality care at the most challenging point in life.
  • See the person behind each prognosis and create care plans centered around the patient’s wishes, needs, and beliefs.
  • Work in concert with the patient’s primary care physician.
  • Manage approximately 45-60 patients in their home settings via an interdisciplinary team approach.
  • Make visits in-person and via telehealth.
  • Participate in Weekly team IDG meetings.
  • Advocate for your patients, discussing goals for their care while addressing their hopes and fears.
  • Live the hospice mission, seeking teachable moments everywhere.
  • Educate patients, caregivers, and the communities you serve on the benefits of compassionate, dignified end-of-life care.
