About the Role
Are you a family medicine physician who wants to practice full-scope primary care in a community that truly needs you? Thundermist Health Center is looking for Family Medicine Physicians to join our team across Rhode Island. We have openings at our Woonsocket, West Warwick, and South County locations — each serving a diverse, largely medically underserved patient population. You’ll manage your own patient panel across the lifespan, provide inpatient hospital services, and work within a team-based, integrated care model that includes embedded behavioral health, care management, and community health workers. Thundermist is a multi-site FQHC serving over 65,000 patients, and we’re committed to delivering high-quality care regardless of insurance status, immigration status, or ability to pay.

Requirements

• M.D. or D.O. with completed residency (minimum 3 years) in Family Medicine
• Board Certification in Family Medicine (ABFM or ACOFP) within two years of start
• Full, unrestricted Rhode Island medical license (or ability to obtain)
• Current DEA registration and Rhode Island Controlled Substance Registration
• Current BLS certification; ACLS, PALS, and NRP as applicable
• Ability to obtain and maintain hospital privileges at Thundermist’s partner hospital(s)
• Eligibility to enroll with Medicare, Medicaid, and commercial payers (current CAQH profile required)

Nice To Haves

• Prior experience in an FQHC, community health center, or safety-net primary care setting
• Bilingual (Spanish/English or Portuguese/English) strongly preferred
• Experience with team-based care models, integrated behavioral health, and value-based care
• Experience precepting residents or medical students
• Eligibility for NHSC or Rhode Island state loan repayment programs

Responsibilities

• Provide comprehensive primary care for an assigned patient panel across the lifespan — pediatrics, adolescents, adults, and geriatrics
• Manage scheduled visits, acute care, chronic disease management, preventive care, and women’s health
• Perform comprehensive health histories, physical examinations, and diagnostic assessments; order, interpret, and follow up on labs, imaging, and testing
• Prescribe medications, including controlled substances, within scope of practice
• Coordinate referred services including specialty care, pharmacy, and ancillary services
• Provide crisis intervention and triage, escalating to appropriate levels of care as indicated
• Provide inpatient hospital services and attending responsibilities at Thundermist’s partner hospital(s)
• Participate in after-hours call and coverage per the established call schedule
• Collaborate alongside behavioral health clinicians, psychiatric providers, nurses, MAs, care managers, community health workers, and social work staff
• Support warm hand-offs, integrated behavioral health workflows, and care coordination for high-risk and complex patients
• Coordinate with community agencies and social service partners to address social determinants of health
• Complete clinical documentation in the EHR (eClinicalWorks) accurately and within established timeframes
• Support HRSA UDS reporting and value-based care performance through consistent care gap closure, screening completion, and preventive care delivery
• Participate in chart audits, peer review, and quality improvement activities
• Maintain practice consistent with HRSA, OSHA, HIPAA, CLIA, and applicable state and federal regulations
• Precept physician residents, medical students, or advanced practice clinicians as assigned
