About The Position

Chestnut Health is a multi-specialty healthcare organization providing rehabilitative care, with a focus on Physical Medicine and Rehabilitation (PM&R) and physiatry. The Physician Assistant will join a collaborative, patient-centered physiatry team and help restore function and improve quality of life for people with injuries, illnesses, or disabilities. The role is open to new graduates and is described as supporting work-life balance. There is no after-hours call.

Requirements

Actively licensed to practice in the state of employment.
Current Controlled Substance License, required upon start date.
Current DEA License, required upon start date.
NCCPA Certificate.
Desire to work in Physiatry/Physical Medicine and Rehabilitation.
Certified Physician Assistant certification is required.

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
Conduct patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine rehabilitation goals and interventions.
Monitor patient progress and adjust treatment plans as needed to optimize functional outcomes and promote independence.
Participate in multidisciplinary team meetings, case conferences, and educational activities.
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical record systems.
Educate and counsel patients and their families about the rehabilitation process, expected outcomes, and post-discharge care.
