About The Position

CAROLINA SENIORCARE IS HIRING A PHYSICIAN ASSISTANT (PA) OR NURSE PRACTITIONER (NP) in Lexington, NC. This role operates on a Monday-Friday schedule from 8:00 AM to 4:30 PM, offering a unique opportunity in healthcare where you can truly get to know your patients. Requirements

• Graduate of an Accredited Physician Assistant Program
• Current North Carolina Certification
• DEA Registration
• CPR Certification - will provide
• Minimum one year working with the frail or elderly population
• Passion for serving older adults
• Team-oriented mindset
• Commitment to person-centered care

Nice To Haves

• Current RN and NP Licensure
• Prescriptive Authority Preferred
• Two years of geriatric experience preferred

Responsibilities

• Provide comprehensive primary care to participants
• Complete assessments, evaluations, and care planning
• Collaborate closely with nurses, therapists, social workers, and caregivers
• Manage acute and chronic health conditions
• Coordinate nursing home care and discharge planning
• Order and review labs, diagnostics, and treatments
• Educate participants and families on health management
• Build lasting relationships with seniors and their caregivers
