About The Position

OPTIM MEDICAL CENTER SCREVEN is seeking a full time Physician Assistant or Nurse Practitioner for our location in Millen, GA. This role provides comprehensive primary care services to patients in a rural health clinic setting under the supervision of a licensed physician and in accordance with applicable state laws and organizational policies. The provider will evaluate, diagnose, and treat acute and chronic medical conditions, promote preventive healthcare, and collaborate with the healthcare team to ensure high-quality, patient-centered care for individuals across the lifespan.

Requirements

  • Graduate of an accredited Physician Assistant or Nurse Practitioner program.
  • Current Georgia Physician Assistant license or Current state registered nurse license/Current state license as an advanced registered nurse practitioner.
  • Current DEA registration (or ability to obtain).
  • Current Basic Life Support (BLS) certification.
  • Strong clinical assessment and diagnostic skills.
  • Knowledge of primary care, family medicine, and preventive health practices.
  • Ability to manage patients across all age groups.
  • Excellent communication and interpersonal skills.
  • Ability to work independently while collaborating effectively with a healthcare team.
  • Strong organizational and time management skills.
  • Proficiency with electronic medical records and clinical documentation systems.

Nice To Haves

  • One (1) to three (3) years of primary care, family practice, rural health, or community health experience.
  • Advanced Cardiac Life Support (ACLS) certification.
  • Understanding of Rural Health Clinic regulations and reimbursement requirements.

Responsibilities

  • Conduct comprehensive patient assessments, including medical histories and physical examinations.
  • Diagnose and treat acute illnesses, chronic diseases, and minor injuries.
  • Develop and implement treatment plans based on evidence-based practices.
  • Order, interpret, and evaluate laboratory tests, diagnostic studies, and imaging results.
  • Prescribe medications and treatments in accordance with state regulations and organizational policies.
  • Perform minor office procedures within the scope of practice.
  • Provide preventive care services, including annual wellness visits, immunizations, and health screenings.
  • Educate patients and families regarding disease management, treatment plans, medications, and healthy lifestyle practices.
  • Collaborate with physicians, nurses, specialists, and ancillary staff to ensure continuity of care.
  • Refer patients to specialists and community resources as appropriate.
  • Participate in chronic care management and population health initiatives.
  • Follow up on test results, referrals, and patient care plans.
  • Accurately document patient encounters in the electronic medical record (EMR).
  • Complete medical records, orders, and other documentation in a timely manner.
  • Maintain compliance with Rural Health Clinic (RHC) regulations, HIPAA requirements, accreditation standards, and organizational policies.
  • Participate in quality improvement activities and performance initiatives.
  • Support the mission, vision, and values of the organization.
  • Participate in staff meetings, provider meetings, and continuing education activities.
  • Assist with community outreach and health education programs as needed.
  • Maintain professional licensure, certification, and continuing education requirements.
  • Provides safe, high-quality patient care.
