Our Cardiovascular Data Management Center provides comprehensive and continuous quality improvement information and allows us to compare our patient outcomes to state and national benchmarks. The APP will be dividing their time between SNF setting. The APP will play a vital role in filling the gaps and needs of our heart failure population, educating our patients and families lifestyle modifications, GDMT and providing an additional resource to the affiliated cardiology practices.
Requirements
- Graduate of an accredited Physician Assistant or Nurse Practitioner program
- Licensed, or eligible to be licensed, as a Physician Assistant, or Advanced Practice Registered Nurse (APRN) in the state of Connecticut
- Certification or eligible to be certified as a Physician Assistant or National Board Certification as an Advanced Practice Registered Nurse
- Cardiology experience required
Nice To Haves
- Geriatric board certification preferred
Responsibilities
- Dividing their time between SNF setting.
- Playing a vital role in filling the gaps and needs of our heart failure population.
- Educating our patients and families lifestyle modifications, GDMT.
- Providing an additional resource to the affiliated cardiology practices.
- Direct support from the Chief of Cardiology to help create a new cardiology consultation program within the Skilled Nursing Facility setting.
- Autonomy to create your own workflow and schedule.
- Only travel between sites located closely together.
