The Advanced Practice Provider promotes and maintains health by providing medical services to Sunrise Community Health patients under the supervision of a Physician. The Advanced Practice Provider identifies short-term and long-term patient care issues that must be addressed, provides information and prescriptions, recommends options and courses of action, and implements Physician directives. The Advanced Practice Provider rotates between clinics for coverage. With a Quality, Customer First, and Compassionate approach, The Advanced Practice Provider-Pediatrics will provide direct patient care in an outpatient setting, diagnose, treat, and prescribe medication. They will work autonomously or in collaboration with other members of the healthcare team. This role involves assessing patient health by interviewing patients, performing physical examinations, reviewing medications, and reviewing medical histories. The provider will manage acute, chronic illnesses and mental health, write prescriptions, order diagnostic testing, and place referrals for patients. They will interpret and evaluate patient data to determine patient management and treatment options, and institute emergency measures and emergency treatment or appropriate stabilization measures in situations such as cardiac arrest, shock, hemorrhage, and convulsions. Documentation of patient care services by charting in patient and department records is essential. Procedures include performing injections, skin biopsies, suturing, casting/splinting, and managing wounds. Instructing and counseling patients by describing therapeutic regimens, growth and development information, discussing family planning, providing counseling on emotional problems of daily living, and promoting wellness and health maintenance are key responsibilities. Providing continuity of care by developing and implementing patient management plans, and providing "On-Call" coverage as scheduled are also part of the role. The provider will review and respond to patient concerns and reviews patient satisfaction surveys. Compliance with FTCA requirements for malpractice coverage, adherence to clinical guidelines and performance of PCMH/QI measures are necessary. Maintaining a safe and clean working environment by complying with procedures, rules and regulations, and protecting patients and employees by adhering to infection-control policies and protocols are required. Complying with federal, state, and local legal and professional requirements by studying existing and new legislation, anticipating future legislation, enforcing adherence to requirements, and advising management on needed actions is crucial. Maintaining professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies are expected. Maintaining up-to-date license, DEA, board certification, and CME requirements for the position is mandatory. Team-based care involves collaboration and continuous partnership with providers, clinical staff, and community resources in a team approach to promote access to appropriate care in the goal of enhancing patient health. Pre-visit team-based planning includes reviewing the daily schedule and chart by care team, documenting instructions on the schedule, typing them into the EHR appointment note, or in the EHR as a flag or clinical list update. Ensuring patient visits are more efficient by identifying that records from transitions of care are available, results from labs or diagnostics are in the EHR, and labs or other services not specified in standing orders protocols are addressed.
